Endoscopy in Children: What Parents Should Know

 

When children face persistent gastrointestinal problems, doctors may recommend a test called endoscopy. For many parents, the word can sound intimidating, but pediatric endoscopy is an absolutely safe procedure that helps doctors diagnose and sometimes even treat digestive problems in kids.

As it travels naturally through the digestive tract, it takes thousands of pictures of the small intestine, which are transmitted to a recorder worn by the child. This helps doctors detect bleeding, inflammation, or other problems in areas that are hard to reach with regular endoscopy.

 

Why do Children need Endoscopy? 

 

A pediatric gastroenterologist may suggest an endoscopy if your child has:

  • Persistent abdominal pain
  • Difficulty swallowing or painful swallowing
  • Frequent or unexplained vomiting
  • Blood in stool or vomit

Preparing a Child for Endoscopy

 

Preparing the child for an endoscopy depends on the type of endoscopy:

 

For Upper GI Endoscopy: The child should not eat for 6 hours or drink for 4 hours before the test, to keep the stomach empty. 

 

For Colonoscopy: A special bowel-cleaning preparation is required to empty the intestines. The pediatric gastroenterologist will provide detailed instructions, which may include a liquid diet and specific laxatives.

The doctor may give specific instructions if required for the child.

How is Endoscopy performed in Children?

 

Anaesthesia/Sedation: Unlike adults, pediatric endoscopy is almost always done under sedation to ensure the child is comfortable and still. It does not require a long recovery time and intubation, is preferred for regular endoscopy procedures. For longer procedures, intubation and long-acting general anaesthesia may also be required. The doctor discusses the procedure and anaesthesia requirements with parents before endoscopy. 

 

In case of older children, endoscopy can also be done without anaesthesia, if the child and parents are willing. 

 

Time taken for Endoscopy: The procedure usually takes 15–30 minutes, depending on the type.

 

What is done during Endoscopy? 

 

  • A small mouthguard is placed to protect the teeth and the scope.
  • The doctor gently inserts the endoscope and examines the digestive tract. If necessary, small tissue samples (biopsies) are taken for lab testing during the procedure. Parents can be assured that this is painless.
  • If a foreign body is present, special instruments can be passed through the endoscope to remove it.
  • For some children who get constrictions/stricture in the GI tract, which could be due to multiple reasons, a gradual dilation can be performed during endoscopy. For this, the pediatric endocrinologist uses specific equipment along with an endoscope. 

 

Is Endoscopy Safe for Children?

In case of a diagnostic endoscopy, immediately after the endoscopy, the doctor discusses the findings and confirms the diagnosis. However, if any biopsies are taken during the procedure, then they are sent to the laboratory for analysis and may take a few days.
